If you're going to do this, at least properly emulate the game. Dying when you're big when landing on a goomba is both bad coding and not how the game even works... This is just SOPer SOPA Soap
Should "pre-load" the commands
Good game, and great purpose, but having to replay it repeatedly just to learn the Simon-says of it detracts from the game play experience.
You've basically made a game around quick-time events, which has already been run into the ground by the gaming industry.
Certainly a good idea
However, it could use some touch ups and the like. It really could do with some design-element changes just to make it a less stressful on the eyes, perhaps with more variations of arrows or what have you (ie more directions, more multi directions).
Nice concept, though, it's good to see something with some degree of new.
This should be under movies, not games
Great animation, and interesting, etc, but this isn't a game, it's a movie with "scene selection"

A bunch of my friends and I were actually ranting about how there's this new influx of art games with no point (like One Chance) that all seem like copies of one another.
This was delightful! :D
The only chance you have is to not bother.
"One Chance is a game about choices and dealing with them."
What choices? There's no implications or motivation. You are basically limited to "spend time with family in face of doom", "try to fix your mistake" or "cheat on your wife"
Those are some pretty serious considerations to make, with some pretty serious consequences.
Work->Roof->Work->Work->Work->Work = cure, sit in park for eternity with no synopsis or even mentioning that the game has ended or what happened?
Real nice job. The only One Chance involved in this game is bypassing this self important, empty and cliche "sequence of events."
Sure, there are ways to get passed your protoplasmic insistence on not letting us play again, but why bother? Anyone who has thoughts provoked by this game simply doesn't think on a daily basis.
Not bad, but could use a few tweaks
1) Distinction between units for matching isn't very defined. This makes for a bit of difficulty, but can be a bit frustrating.
2) Matching sprites should upgrade. You don't really have cause or reason to look up top most of the time, so you don't get to see the upgrades.
3) Upgrades should be a bit more distinct, instead of the same alteration to each sprite, this could also be used as a gradual way to alleviate 1)
Also I like the music

Well done, but the "rpg" elements of the game...
... are a bit lacking.
I completed the entire game using nothing but +1 power/+1 agility per level and 2 command badges with little difficulty.
The items don't really have much use, overall, and the stats aren't specifically that useful except for particular encounters to avoid status effects.
That and simple game design aside, very well done.
